What to bring

  • Photo ID and your commercial driver’s license
  • Glasses or contacts if you wear them to drive, and hearing aids if you use them
  • A list of every medication you take, with doses
  • CPAP compliance report if you use a CPAP machine (30 days or more)
  • Any letters from your doctor about blood pressure, diabetes, or heart conditions

What the $70 covers

The full FMCSA DOT physical: vision and hearing checks, blood pressure and pulse, the urinalysis for protein, sugar, and blood, and the examiner’s review of your history and medications. If you pass, you leave with your Medical Examiner’s Certificate in hand and your result is entered in the FMCSA National Registry the same day. The $70 is paid when you check in. Nothing is billed later.

If something needs a closer look, such as blood pressure over the limit or a missing CPAP report, the examiner tells you exactly what to bring back. You are not charged twice for the same exam.

Why drivers come here instead of urgent care

This clinic sees drivers only on these days. No sick patients ahead of you, no triage line, no two-hour wait for a 30-minute exam. You are seen in the order you booked, the examiner does DOT physicals all day, and the certificate is printed before you leave. Most drivers are back in the truck in under an hour.

Who this is for

Any driver who needs a new or renewed DOT medical card: owner-operators, company drivers, school bus and transit drivers, and anyone whose certificate has expired or is about to. An expired card is common and easy to fix; the exam is the same either way.

DOT physicals are not covered by health insurance. Payment is collected at check-in, cash or card. If your company has an account with us, bring the company name.

Driving in

From Charlotte: US-74 East to Monroe, about 30 to 40 minutes from Uptown, 25 from Matthews.

From Indian Trail, Waxhaw, Wingate, or Marshville: 10 to 20 minutes.

From Union County, Anson County, or Lancaster, SC: under an hour.

Roosevelt Blvd is US-74 Business. Look for Keith Clinic of Chiropractic on the north side; we are in Suite A.

Questions drivers ask

When are you open?

Every Friday and Saturday through October 31, 11:30 am to 5 pm. Call 1-844-708-5403 to pick your day.

Do I need an appointment?

No, but appointments go first. Walk-ins are seen in the next open slot. Calling ahead means no wait.

Is the urine test a drug test?

No. The DOT physical urinalysis checks for protein, sugar, and blood as a health screen. It is not a drug screen. Drug testing is a separate service and is not part of this exam.

How long is the card good for?

Up to 24 months for most drivers. Conditions such as high blood pressure, diabetes, or sleep apnea can shorten the certificate to 12 months, 6 months, or 3 months depending on control.

What if my blood pressure is high?

You are not automatically disqualified. If your reading is above 139/89 and up to 159/99, expect a 3-month certificate. The examiner can issue a longer certificate at his discretion depending on your history and control. Bring your medication list and any recent readings. Skip caffeine and nicotine for an hour before the exam.

Do I have to send my card to the NC DMV?

No. Your exam result goes into the FMCSA National Registry and the NC DMV pulls it from there. You do not mail or upload anything. Keep the paper certificate in the truck as your proof until the DMV record updates.
